IVP at the Beach House Waiver, Release & Indemnification waiver

ALL VOLLEYBALL participants, CORNHOLE PLAYERS, and individuals participating in YOGA, shall indemnify and hold harmless IVP from and against all claims, liabilities, causes of action or other legal proceedings stemming from claims of negligence or any other claim in tort or contract, by any player, for damage to property, injury or death of any person or persons in any way arising out of, connected to, or resulting from volleyball in a confined space which might include but not limited to airborne/direct contact diseases, running into equipment, furniture, or other players, as well as, being hit by volleyballs, or other flying object, without first signing a Waiver, Release & Indemnification Agreement.

During the course of your time at IVP at The Beach House you may have pictures taken of yourself. These pictures are property of IVP at The Beach House and by signing this waiver you are agreeing to allow us to use these pictures in IVP marketing and advertising materials. 

IVP rules & policies

Read the following thoroughly. It is your responsibility to understand our rules and policies and to follow them while at IVP at The Beach House. A failure to do so will result in being asked to leave the facility and no refund will be extended. 

NO SMOKING OR VAPING 

I understand IVP at The Beach House is a smoke free facility.  By checking this box, I am agreeing to not SMOKE OR VAPE in the facility or on the sand.

Smoking/Vaping is only allowed in the front (east side) of the facility on the smoking deck.

children & minors

If you are planning to bring children, for the safety of everyone, they must remain with you or, if you're playing volleyball, they must be sitting at one of the two tables designated for your court.

 

Children under the age of 10 must be within arm’s reach of an adult (18 + yrs) at all times.

Children are not allowed dig holes in the sand. 

​

Know our kid friendly hours.

Sunday & Mondays (Family Night): All ages, all hours.

Tuesday-Saturday: All ages until 7 pm. 

                                  18+ 7pm-10pm.

                                  21+ 10pm - Close

NO outside food or drink 

Outside food and beverages are not allowed inside IVP at The Beach House. If you wish to bring a water bottle it must be EMPTY upon arrival.*

leave wrist bands on

  • You must be wearing a wristband at all times to play volleyball .

  • If you "lose" your wristband you will have to pay for a new one.  

  • Wristbands may not be swapped or exchanged amongst individuals.

Unsportsmanlike conduct

  • Unsportsmanlike conduct is not tolerated at IVP at the Beach House. Arguing with referees or staff of the Beach House, unnecessary cursing and aggressive behavior are not acceptable.

  • Teams with teammates who exhibit unsportsmanlike conduct my be kicked out of league play. NO REFUND WILL BE ISSUED.

By signing below, you agree that you have read and agree with the terms of the waiver and that the information you provided is accurate. You furthermore agree that your submission of this form, via the 'I Agree' button, shall constitute the execution of this document in exactly the same manner as if you had signed, by hand, a paper version of this agreement.

 

If you do not agree with or cannot perform any of the requirements above, do not sign this waiver and ask to speak with a manager to discuss your personal needs.
