IVP Colorado Waiver, Release & Indemnification waiver
ALL volleyball/pickleball players, shall indemnify and hold harmless IVP from and against all claims, liabilities, causes of action or other legal proceedings stemming from claims of negligence or any other claim in tort or contract, by any player, for damage to property, injury or death of any person or persons in any way arising out of, connected to, or resulting from volleyball/pickleball in a confined space which might include but not limited to airborne/direct contact diseases, running into equipment, furniture, or other players, as well as, being hit by volleyballs, without first signing a Waiver, Release & Indemnification Agreement.
During the course of volleyball play, IVP may take pictures for advertising and marketing purposes. These pictures are property of IVP and by signing this waiver you are agreeing to allow IVP to use these pictures and your image, if captured, in IVP marketing and advertising materials.
Unsportsmanlike conduct will not be tolerated by any IVP player. Unsportsmanlike conduct will result in the removal of yourself as well as your teammates removed from league play and NO REFUND WILL BE ISSUED.
By signing below, you agree that you have read and agree with the terms of the waiver; that the personal information you provided is accurate; that your submission of this form, via the 'I Agree' button, shall constitute the execution of this document in exactly the same manner as if you had signed, by hand, a paper version of this agreement.
