NOTE: If your an individual/solo player looking to join a team CLICK HERE and register as a FREE AGENT.   Do Not Fill Out This Form.

NOTE: If you are a volleyball player and plan to bring children under the age of 12 with you to your league night, know that they must be within arms length of an adult at all times.  You cannot play and leave your child unattended. This rule is strictly enforced.  

NOTE: All IVP teams play on a rotational basis. That means the first week your game may start at 6 pm, another week you may start at 7 pm and another week 8 pm. Sunday Beer League begins at 1 pm.

 

It is your responsibility to check the website for weekly schedule times/courts.

 

THE PERSON FILLING OUT THIS FORM WILL BE CONSIDERED THEIR TEAM'S CAPTAIN, UNLESS OTHERWISE SPECIFIED, AND AS SUCH WILL HAVE ADDITIONAL RESPONSIBILITIES AND DUTIES , INCLUDING BEING RESPONSIBLE FOR THE TEAMS BEHAVIOR.
