NOTE: If you are a cornhole player and plan to bring children under the age of 12 with you to your league night, know that they must be within arms length of an adult at all times. You cannot play and leave your child unattended. This rule is strictly monitored.
THE PERSON FILLING OUT THIS FORM WILL BE CONSIDERED THEIR TEAM'S CAPTAIN AND AS SUCH WILL HAVE ADDITIONAL RESPONSIBILITIES AND DUTIES, INCLUDING BEING RESPONSIBLE FOR THE TEAMS BEHAVIOR.
